Liability Release Form
I understand that participating in training, exercise, athletic development, or any related Body Of Castille activity may include actions or tasks that could be hazardous to a participant.
By signing below, I assume any risk of harm or injury that may occur to me or any minor child included on this family profile as a result of participation. I release Body Of Castille, its owners, trainers, representatives, and agents from liability, costs, claims, and damages that may arise from participation, except where prohibited by law.
If a participant is a minor, I confirm that I am the parent or legal guardian authorized to provide consent. I authorize Body Of Castille to seek emergency medical treatment when reasonably necessary and accept financial responsibility for related costs.
This release applies to the account holder and children identified in the participant list at the time this waiver is signed.
